Suffolk GP Federation
Our practice uses Suffolk GP Federation to support us to deliver some of our services such as providing appointments when our practice is closed or community-based services.

Freedom of Information
Information about the General Practitioners and the practice required for disclosure under this act can be made available to the public. All requests for such information should be made to the Governance team via the Service Manager.
Access to Records
In accordance with the Data Protection Act 2018 and Access to Health Records Act, patients may request to see their medical records. Such requests should be made through the Governance team via the Service Manager. However, patients have the right to ask any member of the practice team for access to their records. No information will be released without the patient’s consent unless we are legally obliged to do so.
